The Park Ridge Chamber of Commerce seeks to recognize outstanding members of the Park Ridge community each year with a Community Star Award and one Lifetime Achievement Award.
Organizations are invited to nominate a worthy individual who demonstrated exceptional community service and other qualities, during calendar year 2012. All Park Ridge-based organizations are invited to submit one nomination.
The nomination form can be picked up at the Chamber office, 720 Garden Street, or sent to you via email.
Nomination forms must be received by email, regular mail, or dropped off at the Chamber office no later than 4:00 p.m. on Thursday, November 8, 2012 in order to be considered.
The awards will be presented at “A Night of Stars” during the Annual Community Star Dinner & Dance on Saturday, January 19, 2013, at Rosewood Restaurant. Awardees and nominating organizations will be notified in advance and shall agree to be present for the award ceremony.
Qualities of a Community Star:
Organizations are asked to nominate a Community Star that exhibits some or all of the following attributes:
· Giver – goes beyond normal duties with extra effort in community building
· Citizenship Role Model – is someone whom others (particularly young people) look up to and can model himself or herself after
· Visionary – makes positive change(s) by introducing initiatives and community-wide programs
· Lifetime Achievement Award Criteria – The Lifetime Achievement Award is presented to an individual whose exceptional and lasting lifetime contributions of community leadership and philanthropic endeavors have improved the quality of life of Park Ridge citizens. This individual may have made one or more outstanding contributions or a series of significant contributions with demonstrable impact. The results of their tireless dedication and devotion leave the indelible footprints of a civic legacy.
